Emil Tanase , D.D.S, Lodi ca dentist ,now accepting new patients form the Lodi ,Galt, Stockton and vicinities.

Initial appointment at Lodi dental office will consist of a Complete Dental Exam ,Full dental X-rays (if required) and dental cleaning or dental prophylaxis .Dr. Emil Tanase will discuss oral dental hygiene  instructions and explain all the dental treatment options and recommend a complete dental treatment plan according to your dental needs .we will work with you and your dental insurance to cover the finances.

Please assist us by providing the following information at the time of your appointment, so we can check insurance eligibility and benefits:

1. Your insurance information, subscriber I.D # or S.S #, Date of Birth of the subscriber /patient.
2. A complete list of medications you are currently taking.

We will give you medical history form/HIPPA privacy form and treatment consent forms to fill up and sign.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to any treatment(i.e. allergies, diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)

RELEASE OF RECORDS /X-RAYS

If you are transferring your records/x-rays from your previous Dentist, we will give you a release form to sign and we will fax it to the previous Dentist for release of record/x-rays .If additional films are necessary, they can be taken at our facility.
